WHAT SHOULD I EXPECT DURING THE PHONE CONSULT?
We will review your New Patient Questionnaire, which needs to be completed prior to your scheduled consultation. If you have recent labs, we can review them and suggest additional or alternative solutions.
HOW LONG IS THE CONSULTATION?
The consultation will last up to 45 minutes.
WHAT DO I DO ONCE I SUBMIT THIS INFORMATION?
Before you are scheduled, our Front Desk Coordinator will contact you to collect some basic information to activate your Patient Portal. This will include your name, contact information, and date of birth.
WHAT HAPPENS AFTER OUR CONSULTATION?
At the end of our conversation, an Action Plan will be provided outlining your personalized health and wellness plan.
